Reworking Classification of First Branchial Cleft Anomalies

In the largest series to date, researchers examined 56 children with first branchial cleft anomalies (BCAs) and proposed a new classification system based on the location of the lesions relative to the parotid gland. They also evaluated the validity of the existing Work classification. The study found that only 55% of BCAs could be classified using the Work method. BCAs within the parotid gland were associated with recurrent infections, scarred tissue planes, lymphadenopathy, and enlarged lymphoid follicles. The recurrence rate was 16%, and not resecting the external auditory canal cartilage increased the likelihood of recurrence. Preoperative imaging was useful in determining the extent of surgery and identifying scarred tissue planes. The study concluded that aggressive surgical resection, including the removal of involved ear cartilage, is necessary to prevent recurrence and associated morbidity.
