Results from a study on rheumatoid arthritis (RA) patients treated with tofacitinib (TOFA) indicated a higher incidence of herpes zoster (HZ) among those with recent biologic synthetic disease-modifying anti-rheumatic drugs (bDMARDs) use. Of 304 RA patients, 97 had recent bDMARDs use, and 207 did not. The overall HZ incidence rate was 5.62 per 100 person-years, with patients in the recent bDMARDs group having a significantly higher HZ risk (incidence rate ratio: 2.34, 95% CI: 1.04–5.19, p = 0.028). Multivariable analysis confirmed recent bDMARDs use (hazard ratio: 2.4, 95% CI: 1.12–4.95, p = 0.024) as an independent risk factor for HZ. Kaplan-Meier analysis corroborated increased HZ risk in patients with recent bDMARDs use (log-rank p = 0.015). These findings suggest that HZ vaccination should be considered for RA patients on TOFA, especially those with recent bDMARDs us
