
The PARAGON-HF trial assessed the effect of sacubitril/valsartan in heart failure with preserved ejection fraction (HFpEF) across different age groups. Participants were divided into three age groups: <65 years, 65-74 years, and ≥75 years. Older participants experienced worse clinical outcomes, including higher rates of heart failure hospitalization and cardiovascular death, compared to younger participants. Despite this, the treatment with sacubitril/valsartan provided similar benefits across age groups. However, older adults were more likely to develop hypotension when treated with sacubitril/valsartan. Age did not affect the treatment’s impact on the primary composite endpoint. Older patients had smaller left ventricular chamber sizes, higher LVEF, and more abnormal diastolic function.
