The study investigated the efficacy and safety of a combined picosecond 755-nm alexandrite laser and topical tranexamic acid (TA) treatment for melasma in 48 patients. Results showed a significant reduction in Modified Melasma Area and Severity Index (mMASI) scores, melanin levels, and red area feature counts in the combination therapy group compared to the laser monotherapy group (p < 0.05). Patient satisfaction was higher in the combination group at 71.4%, versus 54.3% in the monotherapy group. No significant adverse effects were noted with combination therapy, while 10.42% of those receiving only laser treatment experienced temporary pigmentation, which resolved within three months. The study concluded that combining laser treatment with topical TA offers superior outcomes in managing melasma.
