
A phase 3 randomized trial compared the efficacy of selpercatinib, a highly selective RET inhibitor, with cabozantinib or vandetanib in treating RET-mutant medullary thyroid cancer. At 12 months, progression-free survival and treatment failure–free survival rates were 86.8% and 86.2% in the selpercatinib group, respectively, compared to 65.7% and 62.1% in the control group. Overall response rates were 69.4% for selpercatinib compared to 38.8% for control. Adverse events led to fewer dose reductions and discontinuations in the selpercatinib group. The findings suggest that selpercatinib demonstrated superior efficacy and safety in RET-mutant medullary thyroid cancer.
