Sepsis-associated acute kidney injury: consensus report of the 28th Acute Disease Quality Initiative workgroup

Sepsis-associated acute kidney injury (SA-AKI) is a common condition in critically ill patients and is associated with adverse outcomes. The pathophysiology of SA-AKI is poorly understood, but microcirculatory dysfunction, metabolic changes, and inflammation have been implicated. Early identification of patients at risk of AKI and timely initiation of supportive measures is crucial for improving outcomes. Biomarkers associated with AKI are being investigated for early diagnosis. Clinical trials have evaluated care bundles and extracorporeal techniques as potential therapeutic approaches. The article provides graded recommendations for managing SA-AKI and highlights priorities for future research.
