
A study has compared the effectiveness and safety of metal stents versus plastic stents in the management of malignant hilar biliary obstruction (MHBO). In a group of 50 patients with unresectable advanced MHBO, 25 received metal stents (MS), while the other 25 received plastic stents (PS). Both groups achieved a 100% technical success rate, but clinical success rates were higher in the MS group (88%) compared to the PS group (76%). Stent migrations were more common in the MS group (48% vs. 16% in the PS group), but the mean time to recurrent biliary obstruction (TRBO) was significantly longer in the MS group (190 days) compared to the PS group (96 days). Multivariable analysis identified the placement of plastic stents as the primary risk factor associated with TRBO. Despite stent migration, the use of metal stents demonstrated a preference for a longer TRBO. This study suggests that, in the context of bilateral side-by-side stent deployment for MHBO, metal stents may offer better outcomes in terms of TRBO, making them a more favorable choice than plastic stents.
