Similarities and distinctions between acetazolamide and sodium–glucose cotransporter 2 inhibitors in patients with acute heart failure: Key insights into ADVOR and EMPULSE

Acetazolamide and SGLT2 inhibitors, commonly used as adjunctive therapy in acute heart failure, do not have sustained natriuresis due to compensatory sodium reabsorption. Trials with acetazolamide showed improved fluid retention signs but no significant diuretic effect or impact on morbidity/mortality. Empagliflozin trials in acute heart failure did not affect dyspnea, urinary sodium excretion, or body weight in the first days but improved health status and reduced heart failure events at later stages. However, similar effects were observed with neurohormonal inhibitors. In-hospital diuretic intensification did not reduce major heart failure risks. These findings suggest the limited influence of immediate diuretic effects on patients' clinical course.
