
This cohort study evaluated how smoking cessation duration affects retear rates after arthroscopic rotator cuff repair. While functional outcomes improved across all groups, retear rates were significantly higher in current and recent smokers. Patients with at least 3 years of smoking cessation achieved retear rates comparable to nonsmokers. Pack-years and cessation duration independently predicted tendon healing outcomes.
