This study compares the effectiveness of sodium-glucose co-transporter-2 inhibitors (SGLT2is) with dipeptidyl peptidase-4 inhibitors (DPP4is) on major liver outcomes (MLO) in patients with type 2 diabetes (T2D) and metabolic dysfunction-associated steatotic liver disease (MASLD). Among 44,651 patients, no significant difference in MLO risk was observed between the SGLT2i and DPP4i groups. SGLT2is did not demonstrate a lower risk of cirrhosis or hepatocellular carcinoma. Clinicians should consider patient conditions and potential benefits when deciding to switch treatments.
