Structured Mobilization for Critically Ill Patients: A Pragmatic Cluster-randomized Trial

In a stepped-wedge cluster-randomized trial across 12 ICUs, a low-cost, multifaceted mobilization intervention was tested to improve recovery among critically ill patients. The intervention included setting daily mobilization goals, interprofessional communication, and performance feedback. The trial involved 12 ICUs with diverse case mixes. While the intervention did not increase overall mobility, it did improve patients' ability to stand before ICU discharge. The intervention was found to be safe, with no significant differences in ICU mortality, falls, or unplanned extubations between the intervention and usual care groups. These findings suggest that a multifaceted mobilization intervention can enhance specific aspects of recovery for critically ill patients.
