A study investigated the clinical and paraclinical features of tuberculous meningitis (TBM) and brucella meningitis (BM) in endemic regions. Seventy patients, including 28 with TBM and 42 with BM, were studied. TBM patients exhibited a higher risk of altered consciousness (2.06-fold) and extra-neural involvement (4.80-fold) compared to BM. Cerebrospinal fluid analysis showed a higher percentage of polymorphonuclear leukocytes in TBM. Neuroimaging indicated elevated risks of hydrocephalus, infarction, and meningeal enhancement in TBM. Additionally, TBM patients had longer hospital stays and higher risk of unfavorable outcomes. These findings stress the importance of careful differentiation between TBM and BM for appropriate treatment strategies in endemic areas.
