The study involving 1,076 pregnant women in Johannesburg, it was found that 83 women (7.7%) had gestational diabetes mellitus (GDM) based on oral glucose tolerance testing . The evaluation of seven point-of-care (POC) glucometers revealed poor performance, with sensitivity ranging from 17.6% to 87.18% and specificity from 62.7% to 99.8%. The area under the receiver operating characteristic curve (AUC) varied from 0.59 to 0.79, indicating moderate to poor reliability. In contrast, laboratory-based fasting plasma glucose demonstrated superior diagnostic accuracy, achieving sensitivity of 94.0%, specificity of 100%, and an AUC of 0.98. The findings suggest that POC glucometers cannot replace laboratory-based measurements for diagnosing GDM.
