
A systematic review and meta-analysis of randomized controlled trials (RCTs) examined the efficacy and safety of combining N-acetylcysteine (NAC) with pirfenidone versus pirfenidone alone for idiopathic pulmonary fibrosis (IPF). Analyzing data from 5 RCTs (398 participants), the study found that the combination may not significantly reduce skin effects (RR 1.26) or mortality (RR 0.35). The evidence also suggests no substantial impact on side effects, severe side effects, gastrointestinal effects, Δ%DLco, ΔFVC, Δ%FVC, and Δ6MWT. Overall, combining NAC with pirfenidone, in comparison to using pirfenidone alone for idiopathic pulmonary fibrosis (IPF), does not result in a decrease in the occurrence of skin effects and mortality.
