UROLOGY

A multicenter-based, randomized, controlled trial compared the safety and effectiveness of radiation-free (RF) RIRS versus radiation-usage (RU) RIRS for managing kidney stones. From August 2020 to April 2022, eligible patients with unilateral kidney stones (≤20 mm) were enrolled in 5 tertiary medical centres and randomized into RF and RU groups. Using flexible ureteroscopes with holmium: YAG lasers, RIRS was performed. The primary endpoint was the success rate, defined as complete stone clearance or residual fragments ≤ 3 mm without symptoms. The secondary was to ascertain RF RIRS's safety. Out of 140 participants, 128 completed the study (RF: 63; RU: 65). Success rates (78% vs. 80%, P = .8) did not significantly differ. High-grade ureter injury rates were comparable between RF and RU groups (RF = 3 [4.8%] vs RU = 2 [3.1%], P = .6).

Thrombus formation decreased with worsening disease severity and bacterial infection, supporting further validation of T-TAS for assessing bleeding-related risks.

PSYCHIATRY

No significant associations were observed with stillbirth, congenital anomalies, or abnormal Apgar scores.

A retrospective study of 66 patients with Budd–Chiari syndrome found that liver stiffness measurement (LSM) effectively distinguished controlled from uncontrolled clinical status, with an AUC of 0.985. An LSM cutoff of 25 kPa achieved 97.5% sensitivity and 92.5% specificity. Spleen stiffness provided complementary but inferior diagnostic performance.

OPHTHALMOLOGY

A newly developed evidence-based guideline provides recommendations for diagnosing, staging, and managing keratoconus in children and adults. Using systematic evidence synthesis and the GRADE approach, it addresses corneal cross-linking, intrastromal corneal ring segments, keratoplasty, and individualized follow-up. Most recommendations remain conditional because evidence certainty is low or very low in several clinical areas.

A prospective study of 38 men with biochemically recurrent prostate cancer found that intermittent enzalutamide monotherapy increased serum testosterone by a median 63%, reaching 832.5 ng/dL. NEUROLOGY

Higher estradiol was associated with lower CSF tau levels, particularly in APOE ε4 carriers, in an Alzheimer’s cohort.

An analysis of 854 women from the European Prevention of Alzheimer’s Dementia cohort found that higher serum estradiol was associated with lower baseline tau levels in cerebrospinal fluid, particularly among APOE ε4 carriers. PEDIATRICS

A nationwide Colombian study identified 4,608 healthcare encounters related to childhood interstitial and diffuse lung disease (chILD) from 2016–2024. Average annual prevalence was 3.32 per 100,000 children, highest among infants and with a secondary rise during adolescence. Reported cases increased annually (IRR 1.022), while declining markedly during the COVID-19 pandemic.
