Study Identifies Predictors of Severe Hypocalcemia Post-Parathyroidectomy in Hyperparathyroidism Patients

A study of patients with primary hyperparathyroidism (PHPT) undergoing parathyroidectomy (PTX) from January 2010 to December 2022 aimed to identify predictors of severe hypocalcemia (SH) post-surgery. Analyzing 46 patients, 15 (32.6%) experienced postoperative SH, 19 (41.3%) had renal calculi, and 37 (80.4%) had osteoporosis. Significant differences in preoperative serum calcium, phosphorus, and bone markers were noted between SH and non-SH groups. Multivariate analysis identified serum phosphorus, calcium, calcium/phosphorus ratio, and osteocalcin as key predictors of SH. These findings suggest preoperative biochemical markers can predict SH risk after PTX in PHPT patients.
