
A study assessed inflammatory cytokines in the vitreous humor of 38 diabetic patients with proliferative retinopathy and retinal complications, treated with either insulin or metformin+glibenclamide. Quantitative analysis of TNFα and interleukin-8 showed that insulin therapy was associated with higher TNFα levels and lower interleukin-8 concentrations, while the metformin+glibenclamide group exhibited the opposite pattern. Notably, the highest concentrations of interleukin-8 and TNFα were observed in patients with vitreous hemorrhage, with a significant difference in TNFα levels in those with retinal detachment (lowest in combined therapy; p=0.03). Interleukins 1β, 6, and 10 were undetectable. The findings suggest variable inflammatory profiles based on antidiabetic therapy and retinal condition
