The meta-analysis of 12 studies (1,370 patients) showed sub-Tenon’s block offers significantly better intraoperative analgesia (SMD −0.53; p < 0.001) than topical anesthesia during cataract surgery. No difference in postoperative pain, but patient and surgeon satisfaction were higher with sub-Tenon’s. Complications were rare. Only 2 of 14 studies had low risk of bias, suggesting cautious interpretation of findings.
