Successful cryptozoospermia management with multiple semen specimen collection

This retrospective cohort study aimed to determine the prevalence of suitable sperm for intracytoplasmic sperm injection (ICSI) in fresh ejaculated samples from men scheduled for microdissection testicular sperm extraction (mTESE). The study found that 9.5% of planned mTESE procedures were canceled because suitable sperm for ICSI were present in the ejaculate. Providing fresh ejaculated samples on the day of mTESE allowed a significant proportion of men with azoospermia to avoid surgery, with satisfactory ICSI outcomes. Short abstinence periods (<24 hours) resulted in better sperm motility for men with cryptozoospermia.
