
This study investigated the outcomes of nonmetastatic prostate cancer (nmPCa) patients with elevated prostate-specific antigen (PSA) levels, known to increase metastasis risk, who underwent radical prostatectomy (RP) or radiotherapy (RT). Using SEER data from 2004 to 2015, 6,177 nmPCa patients with PSA ≥50 ng/mL were analyzed; 27% had PSA ≥98 ng/mL. Over a median follow-up of 113 months, 5-year and 10-year cancer-specific survival (CSS) rates were 92.3% and 81.5%, respectively; overall survival (OS) was 61%. For PSA ≥98 ng/mL subgroup, 5-year and 10-year CSS were 89.2% and 76%. Factors associated with improved CSS included ISUP grade, N stage, RP treatment, and age. High PSA levels shouldn't exclude radical local therapy.
