Therapy options for Chronic Myeloid Leukemia (CML) patients who fail response milestones remain a topic of debate due to associated risks. A study of 1342 newly diagnosed CML patients examined long-term survival after reaching or missing response milestones. Results revealed that patients failing milestones (e.g., >10% BCR::ABL1IS at 6 months) had a 10% lower survival rate than responders, suggesting a need to revise these milestones. Age didn't significantly impact survival, but it influenced hazard ratios and CML-specific survival. Switching to alternative therapies did not alter the main findings. This study provides insights for individualized treatment decisions.
