Symptomatic Irreversible Pulpitis: The Bacterial and Inflammatory Culprits

The study aimed to compare the levels of culturable bacteria, endotoxins, TNF-α, IL-1β, and substance P in teeth with symptomatic irreversible pulpitis (SIP) and vital normal pulp (VNP) tissues. Samples were collected from 20 teeth with SIP and 12 teeth with VNP tissues for microbial and immunological analysis. The results showed that culturable bacteria were only found in teeth with SIP, while no positive cultures were observed in the VNP group. Teeth with SIP also presented higher levels of endotoxins, TNF-α, and substance P than those with VNP tissues. However, the levels of IL-1β were similar in teeth from both groups.
