Systematic review of intraoperative corticosteroid injections and the risk of infection in arthroscopic surgery

This review addresses the growing use of intraoperative corticosteroid injections (IOCSIs) to reduce postoperative pain in arthroscopic surgery despite concerns about increased infection risk. Analyzing 8 studies with data from over 700,000 patient records, the review found that all 7 retrospective studies demonstrated higher infection rates with IOCSIs, while a small randomized controlled trial showed no infections. The combined odds ratio of infection rates in comparable studies was 2.23 (95% CI: 1.66–3.11). The conclusion emphasizes that IOCSIs more than double the risk of postoperative infection, urging surgeons to carefully weigh the minor clinical benefits against potential complications.
