Systematic Review of the Diagnosis and Management of Necrotising Otitis Externa: Highlighting the Need for High‐Quality Research

The article presents a systematic review of clinical studies on Necrotizing Otitis Externa (NOE) to inform best practices for diagnosis and management. The search covered various databases and included 70 articles with 2,274 patients. Most of the studies were retrospective case series with low methodological quality, and case definitions varied widely. The results showed that most patients were elderly males with diabetes, had otalgia as the main symptom, and were infected with Pseudomonas aeruginosa. Optimal imaging methods for diagnosis or follow-up were unclear, and there was no definitive evidence for optimal antimicrobial therapy duration. Surgery was performed on 21% of patients with no clear indication or procedure. The 1-year disease-specific mortality was 2%, and treatment failure and relapse rates were 22% and 7%, respectively. The conclusion highlights the lack of robust data to support best practices for NOE and the need for a minimum set of reporting requirements and a consensus case definition to facilitate high-quality research.
