This prospective study compared systemic anti-tumor immune responses between irreversible electroporation (IRE) and robot-assisted radical prostatectomy (RARP) in patients with intermediate-risk localized prostate cancer. IRE significantly reduced systemic regulatory T cells and increased activated cytotoxic T cells, enhancing prostate cancer-specific immune responses. In contrast, RARP induced temporary immunosuppression. IRE's effects correlated with tumor volume and suggest potential synergy when combined with immune checkpoint inhibitors like CTLA-4.
