Past randomized clinical trials in urologic chronic pelvic pain syndrome (UCPPS) have struggled due to heterogenous patient profiles and non-specific methodologies. The Multidisciplinary Approach to the Study of Chronic Pelvic Pain suggests phenotype-driven eligibility, symptom-specific outcome measures, and mechanisms-based therapies. Future success may rely on adaptive and sequential trial designs that better align treatment to dominant symptoms and patient subtypes.
