
A study suggests that bone mineral density (BMD) may not be sufficient for predicting osteoporotic fracture risk in postmenopausal rheumatoid arthritis (RA) patients on glucocorticoids. Trabecular bone score (TBS), assessing bone microarchitecture, emerges as a valuable alternative. In a study involving 64 postmenopausal RA patients, TBS values below 1.24 were linked to higher accuracy (79% sensitivity, 84% specificity) in detecting osteoporotic fractures. Combining TBS with a BMD T-score of -2.5 or less improved fracture risk discrimination compared to BMD alone. TBS may serve as a supplementary tool in identifying postmenopausal RA patients at heightened risk of osteoporotic fractures.
