The Acute Effects of Prostacyclin on Right Ventricular Contractility and Pulmonary Artery Coupling

inhaled iloprost increases right ventricular (RV) contractility acutely, the authors of this article argue that prostacyclin-related differences in RV contractility and RV–pulmonary artery (PA) coupling depend on the stage of disease. They provide evidence from a previous study on the acute effects of treprostinil on RV function in 25 treatment-naive patients with pulmonary arterial hypertension (PAH), which showed significant heterogeneity in RV contractility and RV-PA coupling ratio, despite a uniform decrease in end-diastolic elastance. However, there was a nearly universal increase in RV ejection fraction, and improvements in RVEF appear to be more closely related to a decrease in estimated ventricular “unloaded volume” zero-pressure volume intercept (V0). The authors argue that their study differed from the iloprost study in various ways, including the method of measurement and the stage of disease of the patients.
