Post-myocardial infarction ventricular septal rupture (PIVSR) is a rare but serious condition that has high mortality rates. Surgical repair is the preferred treatment, but it is associated with high risks. The timing of surgery is a controversial issue, and several mechanical circulatory support (MCS) devices have been developed to delay surgery until the optimal timing. The Impella and venous arterial extracorporeal membrane oxygenation (VA-ECMO) are two new MCS devices that have been recently developed. When left and right heart failure and/or respiratory failure occur in cardiogenic shock or PIVSR after acute myocardial infarction, a combination of Impella and VA-ECMO called ECpella therapy is used. This review discusses the basic concepts of MCS in PIVSR treatment strategies, the role of ECpella as a bridge device, and the efficacy and complications of ECpella therapy, and timing of surgery.
