This systematic review examines 112 articles (2013–2023) exploring the impact of paternal obesity, metabolic disease, and interventions on male fertility. Findings reveal a negative correlation between obesity, diabetes, and metabolic syndrome with semen quality, sperm DNA integrity, and live births via natural or assisted conception. Lifestyle interventions, varying exercise types, and some pharmacologic and surgical approaches show potential to positively influence male reproductive health. This comprehensive review emphasizes the need for further research to refine clinical guidelines and encourages proactive interventions for enhanced male fertility.
