
A study of 5,666 acute coronary syndrome (ACS) patients undergoing drug-eluting stent (DES) implantation explored the impact of ticagrelor versus clopidogrel in preventing thrombotic events. Patients were grouped based on intravascular ultrasound (IVUS) guidance and P2Y12 inhibitor use. IVUS-guided cases showed a trend toward reduced major adverse cardiac events (MACE). Ticagrelor demonstrated superiority in preventing major adverse cardiac and cerebrovascular events (MACCE) but was associated with increased bleeding. Results suggest that IVUS-guided implantation may mitigate the ischemic benefit difference. Multicenter, randomized studies are recommended for validation.
