Tight Glycemic Control, Inflammation, and the ICU: Evidence for Heterogeneous Treatment Effects in Two Randomized Controlled Trials

The study evaluates the association between hyperglycemia and mortality in critically ill patients and the conflicting results of trials testing tight glycemic control (TGC). The study involves a secondary analysis of two randomized controlled trials to test for a differential response to TGC according to the degree of inflammation. They found that patients with elevated inflammation had lower mortality when treated with TG80–110, whereas patients with lesser inflammation did not. This study raises the possibility of using systemic inflammation as a predictive enrichment tool for response to TGC in the ICU.
