TOS Offers Better Survival Than RT for Elderly HPV-Related OPSCC

The study revealed that elderly patients with HPV-related oropharyngeal squamous cell carcinoma (OPSCC) who underwent primary transoral surgery (TOS) had a higher 5-year overall survival rate (81.6%) compared to those treated with primary radiotherapy (RT) (70.7%). After adjusting for confounders, TOS showed a significantly lower hazard ratio of 0.64 (p < 0.001). The proportion of patients undergoing TOS declined over time, particularly in 2020. These findings suggest that TOS may offer better survival outcomes than RT in this patient group.
