
A recent study sheds light on the growing number of youth with complex care needs (CCN) surviving into adulthood and facing disorganized transitions from pediatric to adult healthcare. Researchers interviewed 15 care providers in Eastern Canadian provinces to understand the current practices and experiences. The findings highlight a shortage of care providers, inconsistent transition timing, and a need for more community resources. Participants recommend a designated transition coordinator, policy implementation, enhanced collaboration among care teams, and the integration of virtual care. These insights aim to improve transition practices, policies, and future research in this critical healthcare area.
