
Transvaginal specimen removal reduces pain and improves cosmetic outcomes. A 12-year study of 285 patients found lower 24-hour VAS pain scores (0.4 vs. 0.8, P < 0.001) and higher 3-month cosmetic scores (4.5 vs. 4.1, P < 0.001) in transvaginal cases. No differences in complications or dyspareunia were observed. Transvaginal extraction offers benefits without increasing surgical risks.
