Thin flap trochleoplasty with MPFL reconstruction significantly improves patella stability and quality of life (QOL). In 63 knees with high-grade trochlear dysplasia, the Banff Patellofemoral Instability Instrument (BPII 2.0) score increased from 29.3 to 71.8 (P < 0.001). The redislocation rate was low (1.6%), and the reoperation rate was 4.8%. No significant predictors of outcomes were identified. At a mean follow-up of 33 months, patients experienced improved QOL and stability.
