
Anti-PD1 and anti-PDL1 therapies are effective in stage IV urothelial cancer, but limited patient response suggests inhibitory factors in the tumor microenvironment. Combining CTLA4 inhibition with these therapies shows promise in locally advanced cases. CTLA4 blockade could enhance peripheral T cell priming, potentially transforming immunologically "cold" tumors into "hot" ones, irrespective of pre-existing intratumoral T cell immunity.
