
This retrospective study assessed the validity of the American College of Radiology Thyroid Imaging Reporting and Data System (ACR TI-RADS) for evaluating thyroid nodules in children. Ultrasound images of 106 nodules were graded based on ACR TI-RADS criteria. Sensitivity, specificity, and predictive values were 81.4%, 53.2%, 68.6%, and 69.4%. An ROC analysis determined a malignancy threshold of >3, correctly categorizing 68.9% of nodules. Two radiologists evaluated images, with moderate agreement. While TI-RADS helps differentiate benign and malignant nodules in pediatric patients with sensitivity and specificity comparable to adults, it doesn't replace the need for fine-needle aspiration (FNA) due to predictive value limitations.
