A systematic review and meta-analysis of seven studies with 444 eyes from 279 healthy participants investigated the effect of the Valsalva maneuver on choroidal anatomy. The maneuver induced a small but significant increase in subfoveal choroidal thickness (SFCT) of 6.5 μm (95% CI: 1.6–11.4 μm, p = 0.01) and a notable increase in the choroidal vascularity index (CVI) by 1.48 (95% CI: 1.23–1.73, p = 0.0002). These findings highlight that the Valsalva maneuver can alter choroidal characteristics, suggesting that patient positioning and behavior should be carefully monitored during macular OCT to ensure accurate choroidal measurements for clinical diagnosis and monitoring.
